fix(claude): rename the Claude plugin ID to macaly-cloud - #7

The product name is Macaly Cloud. The Claude plugin ID was still macaly-code. The Claude marketplace entry, the Claude plugin.json, and the Claude install steps in README.md now use macaly-cloud. The Codex and Cursor plugin IDs stay macaly-code. The plugins/macaly-code folder and the repository name stay the same. Current Claude installs of macaly-code@macaly do not update to the new ID. Users must install macaly-cloud@macaly again.
The Claude plugin connects to a remote MCP server. The plugin directory checks in claude plugin validate warn when such a plugin has no privacy policy URL. The Claude plugin.json now sets privacyPolicyUrl to the Macaly privacy policy, the same URL as the Codex plugin.
