Fix: documents may grow to 40 KB instead of 32 KB - #1187

The busiest guides (DEVELOPER_WORKFLOW, ADD_A_MODULE) sat at the 32 KB limit, so any added sentence bounced a pull request until something else was cut. The ratchet stays: a document over the limit may only shrink. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

The ratchet stays as it is: a document over the limit may only shrink, and a new one may not start over it, so docs still can't balloon. The guides that state the number (AGENTS.md, docs/README.md, DEVELOPER_WORKFLOW) say 40 KB, and the 25 October decision record notes the change.
