Master release stopped: changelog fragment from #4084 is in a subdirectory #4105

The Push run for c280b85 (merge of #4084) failed in Update versioning: .github/bump_version.py reads only files directly in changelog.d/, and #4084 added its fragment as changelog.d/added/4083.md, so the script reported "No changelog fragments found" and exited 1. No version was bumped and nothing deployed.

Fix: rename the fragment to changelog.d/report-live-since.added.md, the flat <name>.<type>.md form the other fragments use.
